The prize will be forfeited if not claimed within 90 days of tournament end

The information I have entered on this claim form is true and accurate

I will return any prize I receive if obtained through cheating, fraud or breach of any rules

I will comply with all reasonable requests for verification of my winning score and all
information included herein

If requested I will do a live demonstration of my playing abilities

Upon claiming the prize, I grant to Games Warehouse and all acting with their authority,
the right to use my name and image in connection with the marketing or disclosure of
winners of any tournament to the general public or otherwise

I release and agree to hold Games Warehouse and all acting with their authority free from
any liability for such use of my name, image and from any other claim that I may now have
against them

By completing this form I show that I have read, understood and fully accept the rules

A given player can only qualify for one national prize per tournament

Employees, employers, relatives, business partners or associated companies of
participating operators and organiser’s forfeit their eligibility to win any prizes

Fraudulent claims will result in prosecution without exception

A player must play a minimum of FIVE games to achieve a threshold score and qualify for
any prizes
